PLEASE BRING THE FOLLOWING TO YOUR SCHEDULED APPOINTMENT:

• MRI, X-ray Films & Reports
• Current Medical Reports
• List of all current medications
• Your insurance cards
• Name, address and phone # of referring & primary doctors
• Referral from your primary care doctor, If required
• Complete patient information forms by registering at Patient Center or down loading the forms.

Accepting New Patients & All New appointments will be accommodated with in 24-48 hours.
